Changes
are planned to become effective in 2008 concerning
the Medicaid Community Care Waiver (“CCW”)
administered by the New Jersey Division of Developmental
Disabilities.
1.
Community Care Waiver-Residential Services Waiver
(“CCW-RS”). To be eligible, you must meet
the ICF-MR level of care and be in receipt of or in
need of residential placement. Services include: individual
supports, day habilitation, supported employment (not
covered by DVRS), respite, community professional
support and training, environmental and vehicle adaptations,
and personal emergency systems.
2.
The Community Care Waiver-Supports Only Waiver (“CCW-SO”).
To be eligible, you must: (a) be living in your own
home or a family home; (b) meet the ICF-MR level of
care; (c) be older than 18 years of age and out of
any educational entitlement; and (d) be in need of
no greater than $64,000 in annual waiver services.
The
main service provided is day habilitation. In addition
to those services provided under the CCW-RS, the CCW-SO
also provides assistive devices and transportation
services. Slots will be limited under this Waiver
(50 for previous Waiver clients and 300 for new Waiver
clients).
3.
The Community Care Waiver-Medically Fragile Children
Waiver (“CCW-MFC”). To be eligible, you
must: (a) meet the ICR-MR level of care; (b) require
medically necessary care; and (c) be 21 years of age
or younger and out of any educational entitlement
for the services requested. Services are limited to
respite, vehicle and home adaptation, and Medicaid
Early and Periodic Screening, Diagnostic & Treatment
(“EPSDT”).
